Renewal Application Form - fillable PDF form instructions

A  Renewal Application is now available for submitting your employee renewal to the MGCC. You have the following options for submitting your information when using this form:

• Print off the form, fill in by hand and mail to the MGCC
• Print off the form, fill in by hand and fax to the MGCC (954-9451)
• Fill in the form online and submit the form by e-mail to the MGCC
• Fill in the form online, print and send by mail or fax
• You may also save the completed form in PDF format for your own records.
