Employee Change Notice Form - fillable PDF form instructions

A new Employee Change Notice Form is now available for submitting your employee information changes to the MGCC. You have the following options for submitting your information when using this new form:

• Print off the form, fill in by hand and mail to the MGCC
• Print off the form, fill in by hand and fax to the MGCC (954-9451)
• Fill in the form online and submit the form by e-mail to the MGCC
• Fill in the form online, print and send by mail or fax
• You may also save the completed form in PDF format for your own records.
